  1. What is the result that you want to achieve, and why do you want this to happen?

  2. For whom do you want to make change?

  3. What may get in the way?

  4. What is a strategy to overcome this obstacle?

  5. How would you rate the importance of this goal on a scale of 1 to 10?

  6. What is your confidence level on a scale of 1 to 10 for achieving this goal?
